Description:           Operator Protective Alarm (OPA)

Genie has determined that the Operator’s Protective Alarm (OPA) installed on any of the models listed on page 6 of the attached document is equipped with a mounting bracket that may become loose. This may cause the OPA assembly to fall off the machine resulting in a collision hazard.

At the time of this report, there are no machine(s) registered under your account that is affected by this Safety Notice. If you have a machine or machines in your fleet within the model and serial ranges listed on the enclosed bulletin please fill out the owner update form attached to the bulletin and return to Genie. You may also visit our website @ http://www.genieindustries.com/registration/index.asp to register your machine.

Genie and ANSI requires that the seller of a Genie machine report to Genie the model and serial number of each machine sold, as well as the name, address, and telephone number of the new owner, within 60 days of the sale. OSHA and ANSI also require that the manufacturer’s campaign bulletins be completed. It is your responsibility to communicate this important information to all machine owners and applicable branches.
